By pressing submit, I am agreeing that I am the parent/guardian of the registrant, and for the use of and participation in/on church facilities or property (Central Baptist Church, Gaffney, SC.), do hereby release and discharge Central Baptist Church and its authorized volunteers and staff, from liability for any injuries occurring out of my child’s use of the facility.  

Furthermore, in case of an accident or medical emergency, if the staff or volunteers are unable to contact the parents/guardians for participants, 17 and under, I hereby grant permission to said staff to administer necessary first aid and/or take registrant to the nearest hospital or medical facility for additional treatment. I understand that I as the parent or guardian am responsible for ambulatory, hospital and medical fees that are associated as a result of receiving the appropriate and necessary medical care. I, the undersigned parent/guardian of the registrant, for the use of and participation in/on church facilities or property (Central Baptist Church, Gaffney, SC), understand and am aware that the recreational activities conducted in and through the church facilities/property involve a risk of injury. I voluntarily allow my child to participate in these activities with the knowledge of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury.  

FOR COLONIAL HEIGHTS BUS PICKUP PASSENGERS ONLY:

I hereby give my permission for all the child listed above to ride the church bus or other assigned vehicles used for church functions to and from the church and the drop-off point at Colonial Heights. I understand that my child will be under adult supervision. I further understand that by hitting the submit button on this form that I am releasing and holding harmless Central Baptist Church of Gaffney, its trustees, employees and volunteers from any liability, past or future, fully and completely. My child is allowed to independently walk to and from the pick-up/drop-off location with my permission, otherwise, it will be my responsibility arrange supervision for my child to and from the  pick-up/drop-off location.
